Overview
Product & regulatory status
Approved uses include kidney transplant rejection prevention and lymphangioleiomyomatosis under the relevant label; longevity is a separate investigational question.
mTOR signaling
Sirolimus inhibits mTOR signaling involved in cell growth and immune responses. The same biological activity underlying research interest also contributes to clinically important risks. [1]
What the evidence shows
PEARL aging trial
The 48-week randomized PEARL trial did not find a significant effect on its primary visceral-fat endpoint. Some secondary or subgroup outcomes improved, which should be interpreted cautiously rather than as proof of longer life. [2]
Earlier safety work
A small placebo-controlled study in older adults evaluated laboratory, immune, physical, and cognitive measures. A short feasibility study can identify some safety signals but cannot establish prevention of aging-related disease. [3]
What remains unknown
Whether use for aging improves disability-free survival or lifespan remains unproven by these studies.
Safety
Important label risks
Sirolimus can increase susceptibility to infection and malignancy through immunosuppression. Label concerns also include impaired wound healing, abnormal lipids, lung toxicity, and clinically important drug interactions. [1]
Prescription formulation and packaging
The cited product is a film-coated oral tablet. Its label says tablets should remain intact and protected from light in their original blister packaging. A different solution or compounded preparation requires its own product-specific instructions. [1]
Understanding product quality
Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [4]
When to seek urgent help
Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [5]
A starting point for discussion
This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.
Common questions
Does an intermittent schedule eliminate its risks?
No. A different schedule does not remove the need for clinician assessment, interaction review, and monitoring. Safety findings apply to the studied participants and product.
Questions for your clinician
Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
