Ask for a written transition plan
Confirm the last dose of the previous medication, when to begin the new one, its exact formulation, and what to do if there is a gap in supply. Ask who to contact if you have symptoms or are unsure about the instructions.
Keep the old medication on old entries
Do not rename historical doses to the new medication. Record each product as it was taken, with the original amount and units. A 5 mg entry for one medicine does not represent an equivalent dose of another.
Make the transition easy to review
Add a dated note explaining that your prescription changed. Keep notes on symptoms, appetite, and any questions around the switch. They are observations, not proof that the new medicine caused a change.
Bring the timeline to follow-up
Review the last few entries from the previous medication and the first entries from the new one. Your clinician can interpret the transition in the context of your health history; Kairo cannot select a replacement medication or schedule.
