Review symptoms by week, without a script

Your log should reflect your experience. There is no week-by-week symptom schedule everyone must follow.

Choose a consistent way to describe severity

Use the same words or scale each time. Add a practical detail such as “missed lunch” or “woke me overnight” so a later entry has context. Record days when you feel well if that is part of your routine.

Mark changes in the record

When your clinician changes the prescribed dose, make sure the next entry uses the new amount. Note other relevant changes rather than attributing everything to medication.

Compare similar windows

A full week of entries and two days of entries are different amounts of information. Notice gaps before comparing them. A blank date is unknown, not automatically symptom-free.

Write down what you want to ask at follow-up. Symptoms that are concerning or interfere with hydration deserve attention now; you do not have to finish a week of tracking before contacting your care team.
