Nicotinamide Riboside

Nicotinamide riboside, or NR, is a vitamin B3-related NAD+ precursor. Human studies distinguish changes in NAD+ metabolism from clinical health benefits.

Commonly sold as a dietary supplement. Supplements are not reviewed or approved by the FDA the way medicines are.

Overview

Product & regulatory status

Evidence for raising NAD+ is stronger than evidence for broad anti-aging benefits.

Precursor rather than direct replacement

NR feeds into NAD+ biosynthesis after metabolism. This is different from administering NAD+ directly, so infusion claims cannot be borrowed from NR studies. [1]

What the evidence shows

NAD+ can rise

A randomized crossover study in healthy middle-aged and older adults found oral NR increased NAD+ and was tolerated during the trial. That establishes a biochemical response in the studied setting. [1]

Metabolic outcomes can be negative

A separate placebo-controlled trial in obese, insulin-resistant men did not find improved insulin sensitivity or whole-body glucose metabolism after 12 weeks. More NAD+ does not automatically mean better glucose control. [2]

What remains unknown

Whether long-term supplementation prevents disability or extends human lifespan remains unresolved by these trials.

Safety

Safety applies to the studied exposure

The clinical trials provide short-term tolerability information, but do not exclude rare events or determine safety for every medication combination or medical condition. [2]

NR is the tested ingredient

The crossover trial evaluated oral nicotinamide riboside. Its biochemical findings cannot establish that an NAD+ infusion or a different precursor has the same absorption and metabolic effects. [1]

Understanding product quality

Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [3]

When to seek urgent help

Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [4]

A starting point for discussion

This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.

Common questions

Can a blood NAD+ result prove it is working for me?

It can show a biochemical change. It cannot by itself show better brain function, recovery, or longevity.

Questions for your clinician

Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
