Metformin

Metformin is a prescription biguanide used to improve glucose control in type 2 diabetes. Its inclusion in longevity discussions should be separated from its established diabetes indication.

Overview

Product & regulatory status

The prescription label supports type 2 diabetes treatment, not an anti-aging indication.

What the evidence shows

Established clinical purpose

Metformin lowers glucose through effects that include reduced liver glucose production and improved insulin sensitivity. The treatment decision depends on diagnosis, kidney function, and the full medication plan. [1]

Exercise research shows tradeoffs

In a randomized study of 53 older adults undergoing aerobic training, metformin attenuated some improvements in insulin sensitivity and aerobic fitness. This finding does not justify stopping prescribed diabetes treatment, but challenges the assumption that benefits always add together. [2]

What remains unknown

Benefits in otherwise healthy people seeking longer life cannot be inferred from diabetes treatment outcomes.

Safety

Lactic acidosis and monitoring

The label warns about rare but serious lactic acidosis, especially with relevant kidney, liver, oxygenation, or mitochondrial problems. Gastrointestinal effects and vitamin B12 reduction are also relevant to ongoing care. [1]

Know the exact formulation

Immediate-release and extended-release products have different product instructions. Kairo education does not replace the label for the formulation actually prescribed. [1]

Understanding product quality

Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [3]

When to seek urgent help

Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [4]

A starting point for discussion

This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.

Common questions

Is it automatically beneficial alongside training?

No. Exercise and medication can interact in complex ways. The reason for the prescription matters more than its reputation as a longevity drug.

Questions for your clinician

Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
