Overview
Product & regulatory status
FDA-approved prescription thyroid replacement products exist. They are not approved for obesity or weight-loss treatment.
T4 supplies thyroid-hormone activity
Levothyroxine replaces T4, which can be converted to active T3 in tissues. [1]
What the evidence shows
Replacement has a defined indication
Synthroid labeling includes primary, secondary and tertiary hypothyroidism, as well as selected thyroid-cancer TSH suppression. Diagnosis and the intended biochemical target differ across these conditions; one isolated symptom cannot identify the correct use. [1]
Subclinical disease is different
TRUST randomized 737 adults aged 65 or older with persistent subclinical hypothyroidism. Levothyroxine lowered TSH but did not improve hypothyroid-symptom or tiredness scores at one year versus placebo. This does not negate treatment of overt hypothyroidism. [2]
What remains unknown
The cause of ongoing symptoms despite treatment may not be inadequate thyroid hormone. Trial results in older adults with subclinical disease do not answer every younger or pregnant patient’s situation.
Safety
Over-replacement
Excess exposure can cause palpitations, arrhythmias, anxiety and bone loss. The label specifically warns against using thyroid hormone for weight reduction. [1]
Absorption and test interpretation
Calcium, iron and several medicines can reduce absorption; biotin can interfere with thyroid tests. Medication and supplement changes belong in the clinical review. [1]
Exact product matters
Tablets and other levothyroxine formulations have product-specific absorption characteristics. A change should be identifiable in the medication record. [1]
Understanding product quality
Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [3]
When to seek urgent help
Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [4]
A starting point for discussion
This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.
Common questions
Does a lower TSH always mean better treatment?
No. The desired result depends on the diagnosis; excessive suppression may represent harmful over-replacement.
What should I report if my numbers change unexpectedly?
Product changes, new supplements or medicines, illness and how the prescription is being taken.
Questions for your clinician
Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
