Commonly sold as a dietary supplement. Supplements are not reviewed or approved by the FDA the way medicines are.
Overview
Product & regulatory status
Oral DHEA is sold as a supplement; FDA-approved vaginal prasterone has a distinct indication and formulation.
Precursor, not a neutral vitamin
Prasterone is metabolized through steroidogenic pathways to androgenic and estrogenic compounds. [1]
What the evidence shows
Two-year aging trial
A randomized placebo-controlled study enrolled 87 older men and 57 older women with low DHEA-related levels. DHEA raised hormone levels but did not produce meaningful improvements in body composition, physical performance, insulin sensitivity or quality of life. [2]
Metabolic follow-up was not supportive
A controlled investigation of DHEA replacement in older adults assessed insulin secretion, insulin action and post-meal glucose handling. It did not establish clinically useful metabolic improvement, so higher DHEA levels should not be equated with better glucose control. [3]
What remains unknown
Long-term prevention of fractures, cardiovascular disease or cognitive decline is unestablished. Results in older adults do not answer every diagnosed adrenal condition.
Safety
Hormone conversion matters
Prasterone can be converted to active androgens and estrogens. Existing hormone treatment and hormone-sensitive conditions therefore belong in the clinical review. [1]
Long-term safety remains incomplete
The two-year aging trial did not identify major adverse effects, but its small groups cannot exclude uncommon or delayed harms. [2]
Approved local indication
Intrarosa vaginal inserts are approved for moderate-to-severe pain with intercourse due to menopausal vulvar/vaginal atrophy, not general anti-aging treatment. [1]
Understanding product quality
Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [4]
When to seek urgent help
Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [5]
A starting point for discussion
This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.
Common questions
Is a low DHEA-S result itself a reason to supplement?
A laboratory result needs clinical context; the aging trial shows that raising it may not improve meaningful outcomes.
Is oral DHEA the same treatment as vaginal prasterone?
No. The route, exposure and evidence are different.
Questions for your clinician
Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
