Overview
Product & regulatory status
Methylcobalamin is sold in supplements and compounded products. US prescription B12 forms identified by NIH include cyanocobalamin and hydroxocobalamin; formulation must be checked.
Coenzyme role
Methylcobalamin is an active B12 form involved in methionine-synthase function. Other B12 forms can be converted within the body. [1]
What the evidence shows
Deficiency and wellness are different
NIH describes B12’s roles and assessment of deficiency, but does not find improved energy or endurance from supplementation in people without a nutritional deficit. Fatigue alone is not a reliable diagnosis of B12 deficiency. [1]
ALS evidence is highly specific
JETALS randomized 130 people with selected early-stage amyotrophic lateral sclerosis to methylcobalamin or placebo. Functional decline was slower over 16 weeks. This intensive disease-specific trial does not establish a wellness injection, prevention strategy or general neurological benefit. [2]
What remains unknown
A trial in ALS should not be extrapolated to brain fog or ordinary tiredness. The best route for an individual deficiency depends on its cause and clinical severity.
Safety
Diagnostic interpretation
Borderline B12 and related markers require context; methylmalonic acid can be elevated with kidney impairment. A single laboratory number can mislead. [1]
Trial safety has limits
The ALS trial’s short controlled follow-up cannot establish long-term safety or benefit of unrelated compounded wellness mixtures. [2]
Name and route matter
A methylcobalamin supplement, a compounded injectable and an approved prescription B12 product should be recorded separately. [1]
Understanding product quality
Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [3]
When to seek urgent help
Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [4]
A starting point for discussion
This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.
Common questions
Is methylcobalamin proven superior to other oral B12 forms?
NIH reports no evidence that supplement absorption differs by B12 form.
What should persistent symptoms prompt?
Assessment of the cause and response to treatment, rather than assuming more B12 will address every symptom.
Questions for your clinician
Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
