Overview
Product & regulatory status
FDA-approved prescription medicine for specified postmenopausal breast-cancer indications; routine TRT estrogen management is not an approved indication.
Blocks estrogen synthesis
Anastrozole inhibits aromatase, the enzyme involved in conversion of androgens to estrogens. [1]
What the evidence shows
Oncology evidence
ATAC randomized more than 9,000 postmenopausal women with operable breast cancer among anastrozole, tamoxifen or their combination. The combination offered no efficacy advantage. These cancer outcomes do not establish benefit for hormone optimization in otherwise healthy people. [1]
Higher testosterone did not protect bone
A one-year placebo-controlled study analyzed 69 older men with low or borderline testosterone. Anastrozole raised testosterone and lowered estradiol, but spinal bone mineral density declined relative to placebo. Improving one laboratory value did not improve every health outcome. [2]
What remains unknown
An isolated estradiol result does not demonstrate that suppressing it will improve symptoms. Long-term risk-benefit for many off-label male uses remains uncertain.
Safety
Bone and lipid effects
The label warns of reduced bone density; elevated cholesterol was reported more often than with tamoxifen in ATAC. Individual baseline risks matter. [1]
Medication interactions
Estrogen-containing treatments may oppose anastrozole’s action, and tamoxifen combination use is not supported by ATAC. A full medication review is relevant. [1]
Oral drug, specific indication
The approved tablet’s oncology evidence should not be presented as proof for compounded combinations or routine hormone-balancing protocols. [1]
Understanding product quality
Compounded drugs are not FDA-approved finished products. Poor preparation can cause contamination or incorrect strength. A catalog entry or a research paper is not a quality assessment of your product. [3]
When to seek urgent help
Severe allergic symptoms such as trouble breathing, throat or tongue swelling, or fainting need immediate help. Call 911 or your local emergency number. This is general emergency information, not a complete list of risks for this compound. [4]
A starting point for discussion
This is not a complete list of adverse effects or interactions. Your health history, other medicines, and the exact formulation matter. Missing safety data is not evidence of safety.
Common questions
Is lower estrogen always better?
No. The male trial demonstrates why reducing estrogen can have tradeoffs even while testosterone rises.
What should be discussed before an off-label use?
The specific symptom target, supporting evidence, bone health and whether alternatives address the underlying problem.
Questions for your clinician
Does the evidence match my condition and this exact product? What risks or interactions matter for me? How would we assess whether it is helping?
